The Captive
An extra in this movie by the name of Charles Chandler was killed in 1915 by a bullet to the head.
It is believed a bullet was left in a gun used by soldiers to give the scene a more accurate effect.
With the recent Alec Baldwin accident, we are sure film sets will be doing triple checks in the future.
