Concrete mixer truck almost slides into foundation

Who do you think is responsible for this mishap?

Each worker has a different role at any job. A well-run team knows their part in ensuring their collective goal is achieved.
However, the team can sometimes fail when one person fails to fulfil their role efficiently. It's like that saying, many hands make light work. If you think about it, successful people only succeed with a team, so it takes many hands to make anything work...
A video shared on social media reminded us of these key factors: how working as a team is vital to getting anything done.
In this example of the lack of teamwork and communication, we see a concrete mixer truck reversing into a lot of land to pour out the concrete foundation.
The concrete mixer truck driver gets too close to the foundation and begins sinking into the soil. We're no experts, but a foreman is usually required to be on-site when building.
The foreman can advise whether the soil is sturdy enough for a 26-ton truck to drive over it; the engineers usually provide this information.
